TELEWORSHIP CORPORATION ONLINE PRIVACY & CPNI POLICIES
TeleWorship Corporation ("TeleWorship") recognizes our customers' privacy concerns and strives to protect the privacy of those who use its communications services. The company maintains strict customer information privacy policies and uses state of the art technologies to safeguard customer information and communications from unauthorized intrusions.
TeleWorship is strongly committed to protecting the privacy of our customers. Therefore, we are providing you with this Privacy Policy that describes how we collect, use, disclose, secure, and retain information about our customers, how you may review and correct errors to that information, and how you may enforce your rights regarding your personal information.
This Privacy Policy applies only to our TeleWorship services and the Web sites of TeleWorship Corporation.
How TeleWorship Protects Your Privacy Online
Collection and Use: TeleWorship will collect and use customer identifiable information for billing purposes, to provide and change service, to anticipate and resolve problems with your service, or to create and inform you of products and services that better meet your needs. This means that TeleWorship may use your customer identifiable information, in conjunction with information available from other sources, to market new services to you that we think will be of interest to you, but we will not disclose your customer identifiable information to third parties who want to market products to you.
When you access the Internet, the computers that host Web sites you visit (Web servers) automatically receive some anonymous information. This "usage data" may include a record of which pages a Web browser has visited. TeleWorship servers receive usage data when customers visit TeleWorship Web sites. TeleWorship may use usage data to provide advertising about goods and services that may be of interest to TeleWorship customers, or to provide customized features and services. TeleWorship will not use information about your activities on the Internet together with any information that identifies you without your consent.
TeleWorship may use various kinds of software devices to collect information about Internet use. Small files called "cookies" may be attached to your Web browser. These files identify your browser and save information such as passwords so that Web sites can recognize you. You can set your browser to disable cookies, but some TeleWorship Web sites (and other Web sites) may not work properly if you do this.
In addition, on some Web sites, TeleWorship may use small bits of code called "one-pixel gifs," or "clear gifs" embedded in some Web pages, to make cookies more effective. TeleWorship will not associate the information these software devices collect with your name or email address.
TeleWorship does not supply customer identifiable information to advertising companies or to individual advertisers or other third parties.
Disclosure: TeleWorship will not sell, trade, or disclose to third parties any customer identifiable information derived from the registration for or use of an TeleWorship online service -- including customer names and addresses -- without the consent of the customer (except as required by subpoena, search warrant, or other legal process or in the case of imminent physical harm to the customer or others). When TeleWorship uses other agents, contractors or companies to perform services on its behalf, TeleWorship will ensure that the company protects your customer identifiable information consistent with this Policy. If TeleWorship includes your name and any other customer identifiable information in a directory that TeleWorship creates from information we receive as an online service provider, the company will give you the opportunity to have your information excluded from that directory. Business directories that TeleWorship creates may, however, contain information obtained from other sources.
Customer Choice: A customer may choose not to receive direct marketing communications from TeleWorship in connection with TeleWorship online services. Upon such choice, TeleWorship (a) will not contact that customer directly with marketing messages about TeleWorship online services, and (b) will not use customer identifiable information obtained from that customer's registration for or use of an online service to contact that customer with marketing messages about any TeleWorship products or services. A customer may choose not to receive such messages by e-mail, by mail, or by telephone. To do so now, please contact us either by email at unsubscribe@teleworship.com with "Opt-Out" as the only text in the Subject: line and from which method of contact you want to opt-out; or by phone at our customer contact center 1 (214) 295-1100.
Customer-identifiable information which a visitor volunteers at one of the TeleWorship Web Sites to order TeleWorship services other than online services will be protected just as if the information had been provided under more traditional ways of ordering that service.
Security: TeleWorship has implemented technology and security features and strict policy guidelines to safeguard the privacy of your customer identifiable information from unauthorized access or improper use, and we will continue to enhance our security procedures as new technology becomes available.
Improper Conduct: TeleWorship may also use customer identifiable information to investigate and help prevent potentially unlawful activity or activity that threatens the network or otherwise violates the customer agreement for that service.
Account Information: TeleWorship honors requests from customers to review all customer identifiable information maintained in reasonably retrievable form, which currently consists of your name, address, e-mail address, telephone number and/or billing information, and will correct any such information which may be inaccurate. Customers may verify that appropriate corrections have been made.

Changes in Practices: TeleWorship will keep this Policy current. The company will inform you of any changes that we make.
Additional Privacy Protections for Children Using the Internet
TeleWorship recognizes that children, including young teens, may not be able to make informed choices about personal information requested online.
Accordingly, TeleWorship does not allow persons younger than eighteen years of age to register for service. Further, we do not target children or teenagers (younger than eighteen years of age) for collection of information online. TeleWorship does not solicit or collect customer identifiable information targeted at children and teenagers under eighteen.
Customer Proprietary Network Information ("CPNI")
The information that we have relating to the (1) quantity, technical configuration, type, destination, location, and amount of your use of the service, and (2) information contained on your bill concerning the services that you receive is subject to additional privacy protections. That information, when matched to your name, address, and telephone number is known as "Customer Proprietary Network Information" or CPNI for short. Examples of CPNI include information typically available from details on a customer's monthly phone bill -- the type of line, technical characteristics, class of service, current phone charges, long distance and local service billing records, directory assistance charges, usage data and calling patterns. (CPNI does not include your name, address, and telephone number, because that information is protected by the general privacy policy described above.)
As a subscriber to our services, you have the right, and TeleWorship has a duty, under federal law, to protect the confidentiality of CPNI. From time to time we would like to use the CPNI information we have on file to provide you with information about our communications-related products and services or special promotions. Our use of CPNI may also enhance our ability to offer products and services tailored to your specific needs. We would like your approval so that we, our agents, affiliates, joint venture partners, and independent contractors may use this CPNI to let you know about communications-related services other than those to which you currently subscribe that we believe may be of interest to you. You do have the right to restrict this use of CPNI. IF WE DO NOT HEAR FROM YOU WITHIN 30 DAYS OF THIS NOTIFICATION, WE WILL ASSUME THAT YOU APPROVE OUR USE OF YOUR CPNI FOR THE PURPOSES OF PROVIDING YOU WITH INFORMATION ABOUT OTHER COMMUNICATIONS-RELATED SERVICES. YOU HAVE THE RIGHT TO DISAPPROVE OUR USE OF YOUR CPNI, AND MAY DENY OR WITHDRAW OUR RIGHT TO USE YOUR CPNI AT ANY TIME BY CALLING THE CONTACT NUMBER ABOVE. (We will also honor any restrictions applied by state law, to the extent applicable. If you do not have access to the Internet, please call the contact number above for information about whether additional CPNI requirements are applicable in your state.)
Any action that you take to deny or restrict approval to use your CPNI will not affect our provision to you, now or in the future, of any service to which you subscribe. You may disregard this notice if you previously contacted us in response to a CPNI Notification and denied use of your CPNI for the purposes described above. Any denial of approval for use of your CPNI outside of the service to which you already subscribe is valid until such time as your communications services are discontinued or you affirmatively revoke or limit such approval or denial.
The CPNI Policy above may be required by law to apply to our services.
